Ascension: Moving Forward Pledge Form

Ascension Catholic Church has entered into an agreement with Community Bank in Oak Park that will allow parishioners to make direct deposit transfers from your checking and savings accounts without having to write checks. You can use this ACH to make your Sunday
donation, pay your tuition and contribute to our Capital Campaigns Ascension: Moving Forward or Providing for Tomorrow. Please complete the form below and send it to the Parish Office. Please complete one form for each donation chosen.
